In many cases, pain with intercourse is related to a hypertonic pelvic floor meaning that the muscles of the pelvic floor are so spasmed that they cannot relax. 

The dominant muscles of the pelvic floor are the levator ani muscles which are made up of the pubococcygeus, iliococcygeus, and puborectalis muscles. Levator ani spasms are a leading cause of painful intercourse and have many treatment options.

Conservative management includes pelvic floor physical therapy and supportive care with ice, heat, and anti-inflammatory medications. However other treatment options are available through a urogynecologist and can include vaginal valium, trigger point injections to the pelvic floor, and even pelvic floor Botox.
